If you do not see a "Thank you for your submission" message and the "please wait" button just hangs there, your submission has not gone through.  It  is usually due to a browser issue.  You can try another browser, clear your browsers' cache, make sure Javascript is enabled and try submitting again.  If any of these suggestions does not remedy the error you can instead download the Pdf version of the form by clicking on the doggie to the right and email it to info@fourpawstolove.org.
